DIRECTIVE

Last Will & Testament

 Sign with required witnesses/notary; keep original safe.

Name Executor

Executors should know the location of documents and contacts.

Transfer on Death (TOD) / Payable on Death (POD)

File TOD/POD forms to avoid probate. Keep copies.

 Financial Power of Attorney (POA)

Ensure durable POA if you want it valid during incapacity.

 Medical Power of Attorney (Healthcare POA / Proxy)

Appoints someone to make healthcare decisions.

Living Will (Advance Directive)

States treatment preferences (life support, ventilator, etc.).

Do Not Resuscitate (DNR) Order (if desired)

 Must be signed by a physician; usually bright-colored form.

Organ Donation Wishes

Can be on a driver’s license or separate form.

 Release of Liability (optional)

Only if gifting property/assets early. Not always required.

Court/Legal Forms (if applicable)

 Varies by state (guardianship, probate, etc.).

Funeral/Burial or Cremation Preferences

Put wishes in writing (not just the will).

Central Binder / Folder

Store originals together; share location.

Digital Backup

 Scan and store in a secure cloud or encrypted USB.

Emergency Wallet Card

 List healthcare proxy, main contact, and location of documents.
